Your Kidneys keep your body clean
make sure they are in good shape
Early detection and prevention of chronic kidney disease.

The kidneys are two bean-shaped organs, each about the size of a fist. They are located just below the rib cage, one on each side of your spine. Healthy kidneys filter about a half cup of blood every minute, removing wastes and extra water to make urine.

Your body is made up of two thirds water and it's job of your kidneys to maintain your hydration level. Your kidneys also maintain your blood electrolyte balance and help flush out toxins from your body. A combination of diet, herbs and good practices will keep them going strong.

Kidney disease is a condition of damaged kidneys when waste & fluids are not getting removed, instead getting accumulated in the body. The severity of kidney disease may differ according to different groups: in India.Over 1,75000 patients are on chronic dialysis and every year about 2,20000 - 2,75,000 new patients need kidney or Renal Replacement  Therapy.

Chronic kidney disease (CKD) is a progressive loss of kidney functioning over a period, months or years. 17% of India's population has chronic kidney disease (CKD). Therefore, screening and early evaluation of kidney diseases are important. Generally caused by uncontrolled diabetes and hypertension. 2.5 lakh people diagnosed with end-stage kidney disease in India each year. Only 10% receive either dialysis or transplant. This is because a shortage of dialysis centres performing transplants, shortage of organs and cost involved. 9000 people in India get a kidney transplant every year.

Change in the lifestyle is the root cause of C.K.D. Our food has been replaced by new hybridized varieties; use of pesticides and chemical fertilizers has further worsened the problem. Methods of preservation have drastically changed. Due to facilities for packing and transportation, every kind of food item is available across the globe and people are drastically turning from traditional food to fast food and junk food. Food is the most important aspect of the lifestyle. This is because our body tissue (अन्नमयकोश) generate from the food and hence food is a matter of concern for health.

These conditions are preventable with simple lifestyle changes such as cutting down salt and sugar intake, exercising regularly, quit smoking.

“Hypertension + uncontrolled Diabetes = Chronic Kidney Disease”

Introduction
Chronic kidney disease (CKD) is a prevalent and serious health issue affecting millions worldwide. often referred to as  a silent killer. (CKD) progresses unnoticed until its advance stages, leading to the need for dialysis or even kidney transplantation. However by focusing on early detection and prevention individual can significantly reduce the risk of reaching such critical point.

The Importance Of Early Detection
Early detection is a pivotal factor in managing (CKD) effectively. Regular health check ups and routine screening play  a crucial role in  identifying kidney dysfunction to its early stages. Monitoring blood pressure, blood sugar levels and kidney functions markers such as creatinine and glomerular filtrations rate (GFR) can help health care professionals identify CKD before symptoms become apparent.

Longstanding disease of the kidneys leading to renal failure. The kidneys filter waste and excess fluid from the blood. As kidneys fail, waste builds up. Symptoms develop slowly and aren't specific to the disease. Some people have no symptoms at all and are diagnosed by a lab test. Medication helps manage symptoms. In later stages, filtering the blood with a machine (dialysis) or a transplant may be required.
Kidneys are two bean-shaped organs, each about the size of the fist, situated above one's waist and responsible for balancing the fluid contents in one's body. They produce an enzyme called renin that helps control blood pressure and a hormone called erythropoietin that helps to make red blood cells and activates vitamin D to maintain healthy bones. The kidneys adjust the levels of minerals and other chemicals to keep the body working correctly.

Each of your kidneys has about a million tiny filters called nephrons. If the nephrons are damaged, they stop working, for a while, healthy nephrons can take on the extra work, but if the damage continues, more and more nephrons shut down.  After a certain point, the nephrons that are left cannot filter your blood well enough to keep you healthy. When kidney functions fall below a certain point, it is called kidney failure and can be life-threatening.

High blood pressure and diabetes are the most common causes of chronic kidney diseases.
High blood pressure exerts pressure on the arteries that supply blood to the kidneys which weaken them over a period thus affecting their ability to function.

The kidneys are called ‘The mother of organs'. Though they are called as a mother, one should take care of them as children as the name itself indicates, KIDney ! Because of heavy continuous functions, they may easily get tired (this is the reason creator has given the standby!) and bring illness to the whole body.
“Consult a Nephrologist
What does kidneys do?
    Make urine
Remove waste products like urea, creatinine, uric acid and extra fluid from your blood.
Elimination and detoxification of drugs and toxins.
Maintaining electrolyte like sodium, potassium and chloride in the blood,
    Help control your blood pressure
    Make red blood cells
    Balance hormones
    Balance body chemicals
    Maintain healthy bones.

A chronic kidney disease is a progressive loss of kidney functions over a period of time, due to uncontrolled blood pressure and diabetes, the most common causes. As lifestyle becomes faster- paced stressful and sedentary. The trick is to control diabetes and strictly monitor creatinine levels in the blood.
Once the kidney stone is removed, the patient also needs to adopt a healthier lifestyle and ensure she/he has followed up sessions with the doctor. If you are prone to low hemoglobin or severe anaemia, for instance, it could be because your kidneys are not producing enough of the hormone erythropoietin which hints at the possibility of C.K.D
Effect of renal insufficiency
In this condition, the kidney may not be able to filter and excrete enough wastes from the blood. When this happens, waste and electrolyte build up in the blood circulation and act like poison. In addition, this condition also leads to the development of other chronic diseases like diabetes, thyroid, blood pressure and infertility by affecting sexual hormones like progesterone or testosterone in women and men respectively. It also causes anaemia due to insufficient production of R.B.Cs.
“Kidneys are filters of your body”

Nephritic Syndrome
The disease of the kidney generally affects preschool children.

A typical case of a child  between 2 and 6 years whose mother finds that he has the swelling on the face , which is more marked on waking up in the morning. She than realizes that  he has not been

Diagnosis

Urine test for routine examination  pus cells, Culture and sensitivity
Glomerular filtration rate: (GFR) How well your kidney cleans your blood? Normal 90-130 it may drop as you get older. A value below 60 indicates kidney damage.
    Urine test: for albumin.
Kidney damage may cause a protein to leak into the urine. This protein is called " albumin". A 24 hours urine test.
Electrolyte measurement: The sodium level in urine and blood examination.  Others include potassium, calcium, magnesium and chloride.
    Blood pressure normal 120/80 mm
    Blood Test for
    Urea 20-40 mg

    Imaging Test:
    X-Ray/ultrasound to rule our stones
CT scan (computed tomography) and MRI (magnetic resonance imaging)

Sodium restriction 4 g per day for kidney patients”.

Causes of Kidney Failure
Kidney failure is not  an overnight occurrence. It is the end result of  continuing loss of kidney function. Kidneys are most commonly damaged due to medical ailments such as Diabetes, High Blood Pressure Etc.

Dialysis :  Why & When
Dialysis is a procedure recommended for a person suffering from around 90% renal failure. When kidneys fail or do not function properly, dialysis can help in performing their roles of  removing wastes & fluid from blood. Dialysis procedure can help the person regain  better mobility and flexibility; improved well being fewer diet restrictions; longer and healthier life and much more. Additionally, urine output  may be normal at times of  initiation of hemodialysis.
You can take dialysis and still enjoy life. Although transplant is best option, patients on dialysis can enjoy their everyday life. Three dialysis per week if advised should be done .
What happen during hemodialysis?
During hemodialysis, blood leaves your body  through the access, travels through and is filtered by the dialysis apparatus, later on return to your body. Regular Biochemical test are advised to make neccasary changes in medications. Adequate care can reduce complications.
How much fluid must be taken?
Daily fluid intake is 600 ml in addition to your urine output. It means if your urine output is 200 ml per day than you can drink up to  800 ml per day. It includes all types of fluids  such as coffee, tea, rasam, dal etc.
Can I eat all types of fruits & Vegetables?
No not at all. You must restrict your food items that are rich in Potassium, sodium, phosphorus which may have deleterious effect on body when consumed in excess amount.
How to take low salt diet?
You can keep 3 grams (half tablespoon) of separately , can add it little by little during eating. You must avoid packed foods items such a meat, yeast, salted chips, biscuits, nuts, popcorn, papad, pickle, chutneys, Fry cashew nuts, soft drinks, health drinks, cocoa etc ate high in sodium content.

11 Ways to keep Your Kidneys Healthy
  • If you don't drink at least ten glasses of water a day, your urine will become concentrated, which harms your kidneys.
  • Drink freshly squeezed lime juice diluted with water first first thing in the morning, other kidney friendly fruits and vegetables and herb juices include, apple, orange, sweet lime, grapes, carrot, beetroot wheatgrass and parsley.
  • Gokshura, an Ayurvedic herbal preparation is prescribed for frequent kidney infections and problems caused by kidney stones. Cystone is good for kidney stones and chandraprabha is prescribed for those with recurrent kidney problems ( if you don't modify your diet these preparation will not give any benefit)
Watermelon
  • Kidney friendly watermelon is rich in electrolytes and is also rejuvenates the kidney.
  • Use sprouted kidney beans, turmeric and garlic as ingredients when you make a soup.
  • Avoid excess tea and coffee, alcohol and tobacco as they are flushed out with kidney and expose them to a high level of acid.
  • Avoid exposure to chemicals found in cleaning products. protect your hands with rubber gloves and wear a mask when using sprays. When coloring your hairs, drink lots of water to help flush out the chemical pigment.
  • Kidney healing foods contain the rejuvenating B group vitamins and vitamin C. Eat pomegranate, watermelon and papaya. Take probiotic or B12 supplements if you have frequent kidney problems. Cranberries or cranberry juice is a popular remedy for frequent kidney problems.
  • When you need to use the toilet do not hold on for too long. That creates problem in long run.
  • Avoid inner wear made of synthetic fabrics. Maintain good personal hygiene.
  • Supplement your diet with calcium rich ragi and eat lots of fruits and lemon when you are consuming an excess of protein. A protein heavy diet is  acidic and in trying to maintain your body's pH balance the kidney use calcium from your bones.
  • Eat cooling foods such as coconut water, rice, watercress, celery, green vegetables cucumber and mung sprouts.

Treatment
Allopathy
    Establish cause
Treat underlying factors: obstruction, infection, diabetes, hypertension.
    No undue physical exertion
Restrict protein to 20-25 mg daily. Avoid animal protein, unrestricted carbohydrates
Boil vegetable discards the water and use them.
Fluids: Restrict to daily output + 500-800 ml. Use diuretics if edema , weight gain, hyponatremia, uncontrolled hypertension.
    Hypoglycemic drugs
    Antihypertensive
    Hypolipidemic drugs
"Do not use thiazides or potassium-sparing diuretics"
For anemia Iron and folic acid supplements
Acidosis: oral soda bi carb 1-2 gm daily TDS. If severe inject soda bi carb 7.5 % 50-100 ml slow IV.
Other measures Aluminum hydroxide and calcium carbonate to reduce acidosis, hypophosphatemia and hypocalcemia. Vit B complex supplement
“No NSAIDS or nephrotoxic agents”.
"Dialysis is a treatment for end-stage renal failure"
Indication for dialysis
    Immunosuppressant drugs: Prednisone.
    Pulmonary edema , encephalopathy, pericarditis.
    Lab- Severe azotemia (creatinine > 10 mg%, severe acidosis, serum K+ >6.5-7 m Eq /I)
Definitive management: Long-term dialysis (either peritoneal or hemodialysis) or a kidney transplant. Whenever feasible transplant is preferable for a better quality of life and lowers long-term morbidly.
Hemodialysis
A way to artificially cleanse wastes and extra fluid from the blood. Blood is pumped out of the body through a machine for cleansing and then returned back into the body.
It is usually needed 3 or 4 times a week and each treatment may last up to 4 hours.
A strict diet must be followed by a variety of medicines and supplements.
Peritoneal Dialysis
A permanent soft tube called a catheter is used to fill the abdomen with a cleansing liquid called dialysis solution.
The walls of the abdominal cavity are lined with a membrane called peritoneum. The peritoneum lets the waste and extra fluid pass from the blood into the dialysis solution. Waste and fluid then leave the body when the dialysis solution is drained.  This is called an exchange.
PD patients must also follow diet restrictions though it is not as strict as Hemodialysis.
Kidney transplant
This is a process where a healthy kidney is placed from another person into the body.
The new kidney takes over the work of the two failed kidneys.
The donated kidney may come from a living donor or from someone who has just died.
Ayurveda
Panchakarma treatment followed by Rasayana (rejuvenating) treatment every year does help.
Only milk diet is best to reduce the swelling instantaneously.
For hypertensive nephropathy which is associated with a lot of stress and depression. A procedure like Shirodhara , full body massage help in this condition. One has to keep a watch on blood pressure and keep under control.
