How a new mother spends the weeks directly after the child arrives lays the foundation of how the family functions for many years to come. The first forty days after the childbirth are considered as a period of rest , rejuvenation and bonding. Care after delivery for the mother (Sootika, Paricharya) सूतिका परिचर्या is important as the pregnancy care.

Giving a new mother the space to properly heal is a gift that ensures she will have the strength and emotional well being to usher her new baby to life. After child birth mother needs rest, calm and recovery time. The calmer the mother, the more natural and harmonized are mother and baby's responses to each other. Child birth is a significance event for the body and mind. The perinatal care aims at easy delivery and a healthy child.

Introduction
Post natal period (सूतिका कला ) is considered to be the time after expulsion of placenta (the supporting nutrition system of the baby) till 1.5 months or till onset of menstrual cycle after delivery. It is the time when the tissues of the body are weak and vata vridhi.(वात वृद्धि) (increase of air element). There are 64 disorders. The disease arising in this phase are considered to be difficult to treat and hence proper care after delivery is very crucial in the physical and psychological health of the mother.
Puerperium refers to the six weeks following childbirth during which the pelvic organs return to the pre - pregnancy state and the physiological changes of pregnancy are all reversed. The duration of puerperium last until 1 1/2 months( usually after which the menstrual cycle starts). The boundary of the room where mother is their with her newborn is strewn all over with mustard seed,Linus seeds and rice grains. They serve as a boundary preventing the entry of insects. Herbal medicines like asafoetida,cuscus,mustard,Linux,garlic and long pepper which dispels mosquitoes, germs are tied in packets and suspended from the top of the beam of the doorframe. The room is disinfected with antiseptic drugs like Khadija ( Acacia catechu) etc.
It is divided into three phases:
- Immediate Puerperium : Within 24 hours of delivery.
- Eary Puerperium : Upto 7 days
- Remote Puerperium : Upto 6 weeks
There can be many complications for the mother such as
- Postpartum depression,
- Infections
- Inadequate milk supply
- Low back pain
- Excess weight gain
- Genital organ prolapse in later life.
If adequate care is not taken after delivery.
As soon as you give birth to your baby, there are lots of things which has to be taken care of. The new baby has to be fed in time, hygiene of baby and your self has to be maintained, giving a gap between two offspring takes a priority. Along with these losing the weight which you have gained during pregnancy also bothers you. Losing weight after pregnancy is a very big task. A delicate balance has to be maintained with diet and exercise since your welfare is directly tied with that of your infant.
Depression : Reduced hormone levels quickly after delivery by experiencing emotional distress can also lead to depression. Approximately 15% of woman experience depression after child birth.,
It takes 6 months after delivery for a woman to regain her normal health and strength.

Sufficient sleep and rest durning post natal period is required by the woman to regain all her energies and going back to normalcy.

It is usual to be constipated after delivery. Laxatives are given to promote bowel. Movement. Every effort should be made to induce the woman to pass urine, within 6-8 hours of delivery. The genital area should be cleaned and protected after evacuation of the bowel.
Diet
To aid this process her diet is designed in such a way that any disorder is prevented in the process of re-establishing the change. Most of the diet suppresses vata( wind) , are immune boosters, saturating,uterine relaxants and cleansers & galactogogues. The diet is basically unctuous and hot in potency.
She is given rice gruelling and ghee as well as milk. Moong dal soup,horse gram, mutton soup,pumpkin, carrot and cucumber.
She should take boiled and warm water. She should drink a good quantity of hot water.
Nourishing drugs and scum are beneficial.
Sudation.
Sleeping in air free space.

Hygiene

After delivery the woman should use unction fomentation as hot water with full alertness for one month. From 2nd to 10th day the vulva should be cleaned with warm water and exposed to the smoke of caraway seeds and fennel seeds.
She should be given bath, preceded by oil massage.(Oil massage after 4th day)
After bath she should be exposed to fumigation .
The bath may be started on first day if she is not weak.
The first bath or shower after delivery should be supervised as the mother is little unsteady.
Placental site is the site of infection. Proper wash of genitals, should be done to prevent infection.. The area should be kept clean and dry and free from infection.
Washing the used clothes and blankets should be done regularly.
The Lochia
The quantity, quality, odour, the method of staining on the diaper and the presence of any abnormality in the discharge should be noted.
During the puerperium, the diaper should be frequently changed, particularly if the lochia is abundant and offensive and the parts washed well and protected..
Eat jaggery for its purifying effect on the uterus.
Dasamularishta in the dose of 1 ounce (30 ml) twice a day should be given.
Period of 6 weeks following the expulsion of placenta is called puerperal period. A restricted diet, rest and a specific mode of life for a mother.
For the first 2-3 days after the delivery a women should be given rice gruel prepared with milk and ghee.
Three to five grams of pippali powder ( piper longum )Pippali moola ( Piper Chaba)
Ginger should be taken with ghee / oil / bone marrow according to her capacity once a day,
After taking this, her abdomen should be massaged with oil and wrapped properly with a clean cloth.
This should be followed by rice gruel prepared with drug of vidarikand mixed with ghee or milk for next three days. ( 6-7) days.
On 7-8 day she should take cooked sail rice with meat soup. Decoction of Shatavari prepared with yava and horse gram should be given considering her strength and digestive power
After twelve nights she should take nourishing diet to boost her strength and digestive capacity. Ghee should be added to her diet.
Sweets and drugs having vaatahar properties ( alleviates wind) should be preferred.
Bala tail a ( oil that gives strength) should be used for body massage.
Breast feeding
After delivery a mother should feed her child.
For Lactationm
Milk and milk products should be taken in plenty.
Five grams of fennel should be consumed 2-3 times a day with a betal leaf.
3 GMs of cumin seed powder and 2 grams of Shatavari powder should be taken with milk.
Milk should be medicated with 3 GMs of vaata ( Ficus Bengalis ) sauwarchala lava a, jaggery and ghee.
Milk boiled with Shatavari, door a and ikshu should be taken two times a day.

Care of the breast
Sufficient care should be taken to keep the breasts clean and have the nipples drawn out.
The nipples require little attention other than cleanliness and attention to fissures if any.
Since dried milk is likely to accumulate and irritate the nipples, cleansing the areole with water and mild soap is helpful after each nursing.
Post natal abdominal care
nursing After expulsion of placenta the woman should be assured and advised to lie on her abdomen. She should be massaged with oil on her back. Sides and lower abdomen and sweating should be induced..
The lax abdomen should be tied with a bandage.
She should be given ghee, vegetable soup or rice gruelling with dry ginger, cumin seeds,rock salt and pepper. This gives the mother a sense of well being. It also encourages a good circulation , lessens the possibility of venous thrombosis and more specifically restores the muscle tone of the abdominal wall and pelvic floor .

Sweet flag or vacha is administered with honey in minute quantities to infant on the 11th and 21 st day of birth. It is believed that this practice makes the child mentally active and talkative.
